How to Enroll Your Child
Student enrollment/registration is completed online.
- To enroll your child at Desert Sage Elementary School, you will need the following:

A birth certificate or a certified copy

An immunization record (see immunization requirements below)

Proof of residency (utility bill, rent receipt, lease agreement, escrow doc., etc.)
- Parent Identification

Legal guardian or custody papers, if applicable
- Official withdrawal form from the previously attended school (grades 1st - 12th)

- Go to the DVUSD Student Registration page to complete your child's online enrollment.
- Make sure to upload ALL supporting documents listed above before submitting the online registration.
Immunization Requirements
The State of Arizona has revised the immunization requirements for preschool, kindergarten and first grade children entering school for childcare. In addition to Diphtheria, Pertussis, Tetanus (DPT) and Polio (OPV), Varicella (Chicken Pox) immunizations, youngsters need to complete a second dose of the Measles, Mumps, Rubella (MMR) vaccine and receive at least the first dose in a series of three for Hepatitis B prior to starting school. Children entering Preschool will need Hepatitis A immunizations (there are two needed) and 3-4 Hib vaccines. Children turning 11 years old, regardless of grade, will need to have a Tdap and Meningitis Vaccine.
Parents and guardians should contact their physician, the Arizona Immunization Program Office (602-364-3630), or their school nurse if they have questions or would like clarification. Copies of the immunization form are available at all district schools. The form includes a place for certification of religious, medical or personal exemptions, if appropriate.
Pupils who lack documentary proof of immunization shall not attend school during outbreak periods of communicable immunization-preventable diseases as determined by the Arizona Department of Health Services or local health department. The Department of Health Services or local health department shall transmit notice of this determination to the school administrator responsible for the exclusion of the pupils.

Open Enrollment Process

WHAT IS OPEN ENROLLMENT?
Open Enrollment is a program enacted by the Arizona Legislature and requires all school districts to allow students the opportunity to select a school of their choice, if classroom capacity permits. This also includes capacity in Special Education and programs offered. DVUSD complies fully with all provisions of the statute.
WHO MAY APPLY FOR OPEN ENROLLMENT?
Any family living in Arizona who would like to attend a school that is outside their home school boundaries may apply for Open Enrollment into a DVUSD school. Please view the School Boundary Maps to determine the school you are applying for is not currently your home school.
Once you have selected a school you would like your child to attend, please make sure to apply for Open Enrollment first, BEFORE registering your student if you are registering for a school you are not zoned to attend.
DVUSD offers parents the opportunity to express their preference for any district school. Students can request open enrollment to attend any school in the district, provided space is available. Parents registering their children at a school other than their home school will be responsible for transportation. Principals at each school will accept open enrollment requests based upon capacity as determined by current enrollment and an anticipated growth margin. To determine your home school, refer to the attendance boundary maps.
- Registration is normally done at the school of attendance. Most school offices are closed during the month of July, and reopen two weeks before the opening day of school. It is especially important for students entering middle school or high school to meet with the school officials to plan their courses as soon as possible.
- Registration for Preschool and Prekindergarten requires prior acceptance into the program before enrollment. For more information, contact Community Education at 623-376-3903 or 623-376-3979.
